What Is a Disability Determination Package?

The Disability Determination Package is a series of forms that your client will need to complete before they can receive payments from ODSP. These forms will need to be submitted to the Disability Adjudication Unit within 90 days of your client receiving the package from ODSP, unless they have a good reason for missing the deadline (e.g., hospitalization, in custody, etc.).

There are 4 forms included in the package:

  1. Forms to be completed by your client
    1. Consent to Release Medical Information
    2. Self-Report

To complete these, help your client make note of their health problems

  1. Forms to be completed by a health care professional
  1. Health Status Report
  2. Activities of Daily Living Index

To complete these, help your client find an approved health professional to bring the forms to.
